Surbiton circumcisions clinic didn't gain adequate consent

A private health service in Surbiton that offers circumcisions has been rated inadequate by the CQC and placed into special measures after inspectors discovered doctors did not always gain adequate consent or record the dosage of anaesthetic given for the procedure.

Transcend Consulting Room, which is situated in a converted residential house in Warren Drive North, Surbiton, was rated ‘Inadequate’ overall, with particular concerns around safety.

Inspectors said the practice, which offers faith and non-faith based cultural circumcision services for all age groups, did not have a “robust system” for checking the identity of patients.

Despite having a protocol that required both parents to sign consent, inspectors say in several cases consent was not sought from both parents, or where relevant, the patient. They also did not record the batch number or dosage of local anesthetics used during procedures.

Inspectors were also concerned about their “significant events system” which did not flag an issue with a patient who came to the practice after a previous circumcision attempt and had to be referred to hospital. 

Staff did not record contemporaneous notes, and the service did not hold training records for all of its staff or minute its meetings. This was in breach of regulations 12(1) and 17(1) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

The practice was inspected in August this year after recently opening in London. The service currently operates in Scotland and Wales. In the future, it hopes to provide consulting rooms to other surgeons who may be covered by the provider’s registration.

It will be inspected again within six months to check if improvements have been made.
